Paperwork Burden Reduction Act: Form 1095-C Employer Notice


IMPORTANT INFORMATION: Tax Form 1095-C Employer-Provided Health Insurance Offer and Coverage

The Paperwork Burden Reduction Act, signed into law on December 23, 2024, eliminates the requirement for employers to automatically send Form 1095-C to covered individuals.

As a result, The University of Texas System will no longer automatically mail Form 1095-C. Please review the instructions below to obtain an electronic copy of your form.

Alternatively, you may receive a hard copy upon request. If you would like to request a hard copy of the form be mailed to you or if you have any questions, please contact hrsp@utsystem.edu or The Office of Talent & Innovation, The University of Texas System, 210 W. 7th St., Austin, TX, 78701, 512-499-4587.

Residents of California, New Jersey, Rhode Island, or Washington, DC

If you live in one of these states, The University of Texas System will mail a paper Form 1095-C to your address on file unless you have opted for electronic delivery.

Residents of Massachusetts

If you live in Massachusetts, The University of Texas System will furnish state form MA Form 1095-C demonstrating coverage.

Former Employees, Retirees, and COBRA participants 

The University of Texas System will mail a paper form to your address on file.
