Destroy Outdated Evidence of Insurability (EOI) Application Forms
Dearborn National has reported to this office that they continue to receive many outdated EOI Application forms that contain an option to apply for Medical insurance. As you know, effective 9/1/2011, EOI approval for late enrollment in the UT SELECT Medical plan was no longer required.
However, during the last full plan year (September 2012 – August 2013), they received a total of 187 EOI Applications requesting Medical insurance. During the first six months of this current plan year, they received a total of 47 Medical EOI Applications. Although the majority of these requests were made using an old EOI Application form, it is interesting to note that some individuals actually used the current form but creatively wrote in the word, 'Medical', drew a box, and checked it on the form to indicate their request.
Please destroy any supplies of the old EOI Application forms that contain the Medical option and replace with supplies of the current form which is available on the SharePoint site. We will continue to communicate to employees and retirees in our Annual Enrollment newsletters that EOI approval is not required for enrollment in the UT SELECT Medical plan.

FY2015 Rates are Finalized
The insurance premium rates for Fiscal Year 2014-2015 have been finalized and are available on SharePoint (in both Excel and PDF formats). Please note that the rates for the following plans have changed: Medical:
- UT SELECT Medical
- UT SELECT Dental
- UT SELECT Dental Plus
- DeltaCare DHMO
- Superior Vision Basic
- Superior Vision Plus
The rates for all other insurance plans will not change for this next plan year. Please contact OEB if you have any questions.
Superior Vision Contact Information Has Changed
Superior Vision management has informed this office that many COBRA applications continue to be sent to their Orange, CA office which was closed earlier this year. We included the following notice in the March 7, 2014 edition of this newsletter to inform you of this change: Superior Vision COBRA Contact Information Superior Vision has announced the closing of their Orange, CA office; therefore, there is a new COBRA contact to be aware of for the UT Vision and Vision Plus plans. The Internal Vendors Contact List and the UT Benefits COBRA Application for Medical, Dental and Vision Coverage have been updated with the new Superior Vision COBRA contact information and are available on SharePoint. Please update your supply of COBRA materials. Although any applications sent to the Orange, CA address are being forwarded to their updated Rancho Cordova, CA address, it delays the application process and the ability of individuals to access their benefits. If you have any questions, please contact any member of the OEB Benefits Team at firstname.lastname@example.org.
New FAX Number for UT Retirement Program
The UT Retirement Program has a new FAX number! Effective immediately, please use 512-579-5016 to fax any UTRetirement Program-related loans or hardships to this office. If you have any questions, please contact Kosima Wall Ketcham at 512-499-4703 or Charles Marrs at 512-499-4662. You may also send your questions to Kosima or Charles at UTRetirement@utsystem.edu .
